REQUIRED DOCUMENTATION
Proof of Age of one of the following
*a valid Canadian passport;
*a birth certificate issued by a Canadian provincial, territorial or federal government agency; or, any identification card (with or without photo) issued by a Canadian provincial, territorial or federal government agency.
Proof of Health Coverage
*a valid Ontario Provincial Health Card.
To help speed up the registration process, you may wish to bring a photocopy of the health card and birth certificate documents as well as bring the completed forms into the Squadron on a Wednesday evening after 1845hr so that the administrative staff can dedicate the time to assist in the enrolment and answer any questions that you may have.
